Your assistance in completing this
questionnaire will help us place the right puppy with
you. To reserve a puppy from an upcoming litter, we
require a $150.00 deposit payable by cashiers’ check
or money order made out to Little "L" Farm or
Lesa Rice, and this completed application. Deposits are
non-refundable and will be applied to the total purchase
price of the puppy. Applications received without a
deposit will not reserve a puppy, but will put your name
on a litter notification list. (Puppies are usually
pre-reserved long before they are born.)
Please
note: If paying through Paypal add $3.00 per $100 since
this is what I pay for this service.
By signing your application, you agree
to have your puppy spayed/neutered by 6 months of age.
Desexing
is required for all puppies. When
you sign this Puppy Application, you are under a
Spay/Neuter Agreement.
You will be notified by
email when we receive your (signed hard copy)
application with deposit. We will keep you updated on
progress. Feel free to contact us anytime! We look
